BOARDING & DAY CAMP AGREEMENT
Dog Owner gives permission to Happy Dog to care for their dog(s). Dog Owner agreess to save and indemnify and hold harmless Happy Dog owners, staff and volunteers against all liability on their part for loss or damage from disease, death, running away, theft, fire, injury to persons, other dogs or property by said dog or other unavoidable causes, damages, and loss to dog(s) so long as diligence and care have been exercised.Dog Owner assumes liability for any and all damages caused by dog(s). Dog Owner understands that Happy Dog provides group boarding and Happy Dog reserves the right to refuse service if dog(s) shows inappropriate behavior prior to admission.
If, during the boarding stay, Happy Dog deems owner’s dog unsafe for group boarding, Happy Dog shall have the right to place dog(s) in another kennel facility of their choice at the Owners expense. In entrusting dog(s) to Happy Dog, Dog Owner agrees that dog(s) are in good health, and that dog(s) have not harmed nor shown aggressive or threatening behavior toward any person or dog at any time.
If, in the discretion of Happy Dog owners, staff, or volunteers, dog(s) needs veterinary attention, Dog Owner gives express permission for Happy Dog to ensure such attention is provided by the veterinarian named by Dog Owner or any reasonable substitution. All veterinary expenses shall be the responsibility of the Dog Owner.
Happy Dog is not responsible for loss or damage to personal items left at the facility. Payment in full is due at time of pick-up.
I have read and understand the terms set
forth above. I agree to abide by all the terms,
conditions and statements of
this Boarding & Day Camp Agreement.
This agreement is valid from date of signature until December 31, 2010, unless deemed invalid by Happy Dog LLC Management.
